Always read the instructions that accompany the registration form carefully. Here, however, is some useful information in completing your registration.
General Requirements:
1. Use only a #2 pencil.
2. Completely darken the oval spaces under each letter and number entered. On the form:
Items 1-3: As directed. Use your full name each time you register.
Item 4: Social Security Number. If you use it once, use it every time.
Item 5: High School Code: 200720
Item 6: Current grade level. PGs and seniors alike use "12th grade."
Item 7: Darken "May" oval and write in "05" and darken those ovals.
Item 8: As indicated. Your call. Answering 'yes' will get you a lot of college mailings. Usually, these mailings have little bearing on your college search.
Item 9: Test-- fill in which one you want to take (for BA students, usually SAT). Test Date -- fill in oval for appropriate date.
Item 10: Please read this section carefully! For December and January tests at Bridgton Academy, fill in 20-240 as first choice and do not include a second choice. For October test at Lake Region High School, fill in 20-135. If you decide to take the test at your old high school, we will help you find the appropriate school code.
Item 11: Do not complete unless you are sure that you are applying to specific colleges. Note: The first four test reports are free. Additional reports are $7.00 each.
Item 12: Optional. Merely provides marketing information about you to the Student Search Service. See pages 5-8 of the bulletin for directions.
Item 13: Your call. Can use your real home phone, BA room phone, or cell number.
Item 14: You may use your home address, but we recommend using your post office box at Bridgton Academy (eg. John Doe, Bridgton Academy, P.O. Box (fill in your box #), North Bridgton, ME 04057.
Item 15: Write (do not print!!) the statement and sign your name.
Item 16: Use the address in Item 14 (above) as your mailing address.
Item 17: Do Not Complete.
Item 18: Optional. Be sure to include expiration date if you are using a credit card as payment.
Item 19: If you are taking the SAT I, ignore. If you are taking the SAT II, indicate the test(s) you plan on taking. Add up the cost of the tests you plan to take and put the cost on the "subtotal" line. Add in the $18 registration and put the total on the "total" line.
Item 20: Fees--Enter all applicable fees on the appropriate lines and then enter the subtotal and total on the box at the bottom of the page. Darken the appropriate ovals.
TO MAIL:
1. If you will be taking cash out to pay for the test, please see Mrs. Kuvaja to withdraw the appropriate amount for the test(s) you are taking.
2. Take that cash to the North Bridgton post office and get a postal money order (you can't send cash to the College Board). The cost for this is approximately $1.00.
3. After double-checking your data on the registration form, be sure to place the appropriate sticker on the front of the envelope you are sending to the College Board (you will find white labels with various months printed on them in the registration packet envelope; use the sticker that corresponds to the month you are planning to take the test).
4. Seal the envelope, give it a kiss for luck, and pop it in the mailbox!
